1. What is the differential diagnosis suggested by clinical evaluation and autonomic testing? A. Diabetic neuropathy: The typical phenotype is that of generalized sensorimotor polyneuropathy with autonomic dysfunction; there is a progressive compromise of both somatic and autonomic nerve fibers. Cardiovascular, genitourinary, gastrointestinal, and sudomotor dysfunction evolve from subclinical abnormalities to severe clinical manifestations.

B. Amyloidosis: AL amyloidosis and transthyretin familial variety may present a generalized autonomic dysfunction, associated with a distal sensorimotor polyneuropathy; also autonomic manifestation can occur isolated.
